North Pointe Dental Center may use and disclose protected health information for treatment, payment, and healthcare operations, including coordinating care, processing insurance claims, scheduling appointments, quality improvement, and required administrative functions.
We may disclose information when required by law, for public health reporting, to comply with court orders, or for other uses permitted under HIPAA.
Any other use or disclosure that requires your authorization will only be made with your written permission.
We are required by law to maintain the privacy of protected health information, provide this notice, and follow the terms currently in effect. We will notify affected individuals as required if a reportable breach of unsecured protected health information occurs.
